1. Provide a brief history of the institution including when and by whom it was founded.


2. (a) What date were the first students attending classes?


(b) For non-public institutions: what is the date that the institution received licensure to operate a postsecondary institution?

(c) Has the institution / training company graduated at least one hundred students from its programs in last six months?

If no, when is the first student expected to graduate from the institution's longest program?



3. Does the institution offer any academic (non-occupational) programs?

If yes, what are these programs?



4. Has the institution changed ownership within the past two years?

If yes, submit a copy of the approval of the change of ownership from the appropriate state licensing agency.



5. Has the institution been denied candidacy or accreditation by a nationally-recognized accrediting agency?

If yes, attach a detailed explanation of the circumstances at the time of denial.



6. Has the institution voluntarily withdrawn its candidacy or accreditation while not in good standing from another accrediting agency?

If yes, attach a detailed explanation of the circumstances at the time of withdrawal.
